Medical Treatment (MT) or Non-Immigrant (O) for Medical Treatment

Purpose of visit: persons who required medical treatment in Thailand and their accompanying helpers (if any)

Requirement documents:

  1. The original passport (must be valid within 6 months from the travel date)
  2. Photograph of the applicant, taken within the past six months
  3. A completed visa application form (download here)
  4. A copy of the passport biodata page.
  5. Financial evidence that you have sufficient funds to pay for your medical treatment in Thailand, such as a bank statement.
  6. Letter from a hospital in Malaysia that you have conditions that specifically require medical treatment in Thailand.
  7. Letter from the relevant hospital in Thailand which would accept you to be its patient.
  8. Proof of relationship with the patient for accompanying helpers (must be immediate family members) 
***ATTENTION***
For citizens of SPECIFIC COUNTRIES who reside in Malaysia
*Please check whether your passport is from one of the specified countries*

Must provide additional documents as follows:

- Three (3) completed visa application forms with (3) photographs (taken within the past 6 months). (mandatory)

– A copy of the permanent resident in Malaysia (PR card) or a copy of a valid Malaysian visa (at least 3 months validity). For E-Malaysian Pass, a photocopy must be certified by the Ministry of Foreign Affairs (Wisma Putra) before applying to the embassy. (mandatory)

Proof of being employed or a student in Malaysia e.g., the employment letter from the company, the certificate of student status from the institute, business license/registration if self-business, or a copy of MM2H registration. The letter must mention the applicant’s name, position and dates of visiting Thailand. ***Please note that the acceptance or job offer is not sufficient***

For self-employed, business license or business registration indicating the applicant’s name.

**For Nigerian citizens, the original Clearance Certificate issued by Nigeria’s National Drug Law Enforcement Agency (NDLEA) or the Letter of Good Conduct issued by the Ministry of Foreign Affairs of Malaysia must be attached.

For minors under 20 years old, please provide additional documents:

– A Copy of a birth certificate or adoption certificate.

– A copy of the parent's marriage certificate, in case of sole custody,  notarized copy of the court order must be provided.

– A copy of passports or ID of mother and father/ legal guardian.

– Original letter of notarized consent for the minor to travel abroad from mother and father/ legal guardian if the applicant is travelling alone.

*** The Royal Thai Embassy reserves the right to reject any visa application if the reasons given are not fully supported and the visa fee is non-refundable.

*** Additional documents may be required as deemed necessary by the Royal Thai Embassy.

***All supporting materials that are issued outside of Thailand and Malaysia must be accompanied by certified English translations and notarized by an Embassy or Ministry of Foreign Affairs only***
